Patient Information: Insurance & Billing Information
Insurance: Please bring your insurance and/or Medicare
cards with you. GAI participates in a variety of managed care
plans
and
insurance programs. We will submit our patient’s claims to their
insurance
carrier
and
Medicare.
We
must be
provided with
necessary and accurate information. As with all insurance policies,
the contract exists between you and your insurance
carrier.
If your insurance
carrier
does not pay your claim within a reasonable time period,
or if the payment is denied in part or in full, you will be
responsible for paying any balance. As a courtesy to our patients,
should this occur, we can make payment arrangements, if necessary.
NOTE: For procedures performed at The GAI Endoscopy Center, the
patient will incur a FACILITY FEE and PROFESSIONAL FEE.
Medicare: GAI participates with Medicare,
which means that we will accept the amount permitted by Medicare.
You are required to pay deductibles and/or co-pay amounts, as
required by your Medicare plan.
Referrals:
Some
insurance plans require a referral from your primary care physician
before GAI can provide treatment.
If so, please
obtain your referral from your primary care physician before
arriving for your appointment. If you are uncertain that you need a
referral, we recommend that you contact your primary care physician
or your insurance
carrier.
Self Pay: For those patients who are not covered by a
health insurance,
GAI requests payment for services as they are
provided. Payment in full is expected for all charges incurred at
the time of the initial visit (50% payment is required prior to
treatment). For your convenience, GAI accepts cash, check,
MasterCard and Visa.
Disability Forms: A processing fee of $5.00, paid in advance,
is required. Because of the time required to process disability
forms, they cannot be completed on an “emergency” or “while you
wait” basis. For planning purposes, GAI requests a 48-hour
turnaround time to process disability forms.
NOTE: If you have any biopsies done, you will be
billed directly from the hospital for the pathology report.
